A geriatric care manager (GCM) is a professional who specializes in assisting older people and their families with problem solving6 identifying and utilizing community services and making long-term care arrangements. GCM?s have educational and professional training in gerontology6 social work6 nursing6 or counseling. A care manager can provide an extensive assessment of an individual?s current level of functioning and develop a plan for resolving problems and assuring that the person?s physical6 social6 and psychological needs are being addressed. Care managers advocate for the needs of the older person6 being there to assist families and elders in making difficult decisions. Every attempt is made to keep or place the older person in the most comfortable6 least restrictive environment6
What we do: Conduct care-planning assessments to identify problems6 eligibility for assistance6 and need for services;
Screen6 arrange6 and monitor in-home help6 or other services;
Schedule and attend medical appointments to assure continuity of care;
Review financial6 legal6 or medical issues and offer referrals to geriatric specialists to avoid future problems and conserve assets;
Provide crisis intervention6 counseling and support;
Act as a liaison to families at a distance6 making sure things are going well.
Carol Franzen6 Geriatric Care Manager6 has a Master of Science in Gerontology6 is a Licensed Master Level Social Worker6 Advanced Practitioner6 and an Advanced Professional Member of the National Association of Professional Care Managers. She has been providing care management services in the DFW Metroplex and surrounding area since 1985. She founded EXCEL Rehabilitation Consultants6 a Texas Corporation 1990. She serves on the board of the Dallas Area Gerontological Society and frequently provides educational programs regarding eldercare issues. |
